Abstract	Patients with cystic fibrosis (CF) are susceptible to respiratory tract infections at an early age; with the most frequent initial isolate being the Gram-positive pathogen Staphylococcus aureus. Subsequently, these patients become chronically colonized with the opportunistic pathogen Pseudomonas aeruginosa, eventually succumbing to pulmonary failure as a result of chronic inflammation of the lung. Furthermore, antibiotic treatment of these S. aureus infections often results in a relapse of infection that is usually clonal in nature. It is unclear what role S. aureus has in the disease progression in the CF lung. In this study, we sought to characterize the interaction between S. aureus and a CF epithelial cell line so that we may start to understand the role S. aureus plays in the disease progression in cystic fibrosis. Using confocal microscopy, we examined the ability of S. aureus to infect and replicate within a cell line derived from a CF patient and its complemented counterpart. We found that S. aureus is able to replicate within the CF cell line but not the complemented wild type cell line. Furthermore, analysis of intracellular trafficking of internalized S. aureus revealed that the bacteria were able to escape from the lysosomal compartment where it freely replicates within the host cell cytosol. Analysis of a panel of genetic mutants present in the lab allowed us to determine that the global virulence gene regulator agr was required for phagosomal escape. Generation of an inducible plasmid expressing agr -regulated toxins allowed us to determine which specific toxins are involved in vesicular escape. Using this plasmid in the agr mutant strain, we show that the secretion of alpha-hemolysin allows internalized S. aureus to escape from the phagosome. The characterization of this S. aureus /lung epithelial cell interaction will allow us to dissect the fundamental defects present in CF cells that result in bacterial colonization of the lung. By understanding the pathogenesis of S. aureus in CF lungs, our ultimate goal is to utilize the above knowledge to extend the life span of patients afflicted with this disease.
