Abstract	Background Smoking is the number one cause of preventable premature death and disease. Social anxiety disorder is strongly associated with nicotine dependence in adults. The aims of this research were to determine whether social anxiety symptoms were associated with smoking in early adulthood (age 18 to 24 years); to determine whether social fear in early adolescence (age 9 to 15 years) predicted smoking in early adulthood; and to describe the order of onset for social anxiety symptoms and smoking. Methods Data were obtained from an ongoing multi-wave cohort study of youth (n=2002) recruited from 14 randomly selected New Hampshire and Vermont middle schools. The 1999 baseline survey administered during middle school included a single question about social fear, defined as worrying other kids didn't like them in middle school. In 2008, the follow-up survey included two validated measures of social anxiety symptoms and questions on smoking. Logistic regression and general linear models were used to predict current smoking, defined as having smoked 100 or more cigarettes in their lifetime and having smoked in the past month. Covariates included age, school enrollment at followup, college attendance, school performance, depression, movie smoking exposure, sibling smoking, parent smoking, parent education, and friend smoking. Models were stratified by sex. Results Young adult females with severe social anxiety symptoms had higher odds of being current smokers than those with mild social anxiety symptoms (OR 2.2 (95% CI 1.02 - 4.7). This effect was especially evident in those whose friends smoked. Females with a history of social fear during early adolescence were more likely to be smokers as young adults (RR=1.43 95% CI 1.02-1.99). Social anxiety symptoms preceded smoking initiation for most participants (72.8% of males and 54.9% of females). Conclusions There was evidence that current social anxiety symptoms and a history of social fear during early adolescence were associated with a higher risk of smoking among young adult females, but not males. Social anxiety symptoms may exacerbate the influence of peer smoking in females. Social anxiety and early social fear may be important factors to consider for smoking prevention and cessation efforts, particularly in females.
